VAGINISMUS
VAGINISMUS
MAIN SYMPTOMS:
Vaginismus is a pathological condition in which vaginal muscle spasm occurs, which prevents a man from entering the penis to commit coitus. In some cases, the spasm comes directly during the insertion of the penis, and the muscles of the vagina contract so strongly that it infringes, with the gradual development of edema and the need for emergency care. In addition, sometimes vaginismus in women is manifested even from thoughts about sexual contact or touching the genitals. Also, with this violation, it is impossible to perform a gynecological examination, since this requires the introduction of a special instrument into the vulva of women, which causes a painful spasm.
The disease belongs to psychosexual disorders, since it most often develops as a result of the impact of a traumatic psychological factor on a girl in childhood or adolescence. However, sometimes vaginismus is the result of functional disorders, in particular inflammatory processes, tumors or abnormalities of the genital tract. At the same time, the treatment of vaginism requires an integrated approach and the plan is developed individually for each of the women with this problem, depending on the reasons that caused it and the symptoms.
THE REASONS
Women can have primary and secondary vaginismus. It is said about the primary in the case when the first sexual contact provoked a spasm of vaginal smooth muscles, as a result of which it was not possible to make it. In subsequent attempts, the spasms recur, therefore the girl with the primary form of pathology is incapable of sex as such. At the same time, she may have the ability to experience a clitoral orgasm, as well as a pronounced libido.
On the secondary form of the violation they say when the woman already had sexual relations with a man (or several), but at a certain period she developed vaginismus.
Usually, the causes of the development of a disorder in women lie in traumatic events related to sexual contacts in the past. This may be rape (in childhood or adulthood), gross defloration, a relationship with a cruel man who hurt her during sexual contact and other traumatic factors.
Often with such a pathology as vaginism, women face different phobias and fears. It is particularly difficult to treat the disease if the woman has a fear of pain, which is accompanied by the first sexual intercourse. Such fear usually arises in childhood and becomes obsessive over time, so when the moment of the first sexual intercourse is committed by a girl, she becomes too tense, which causes a painful muscle spasm. A disorder can also develop in girls who have received improper or insufficient sex education. Such girls consider sex to be shameful, and experience physical and psychological stress in a situation when they do it, which is why nothing happens.
Sometimes this violation is a defensive reaction. In particular, if a woman is not attracted to the sexual partner, but sexual contact due to some reason must occur, her body can react with just a spasm of the vaginal muscles, trying to prevent coition.
The following physiological factors can provoke this violation:
- inflammatory processes in the vagina, including STDs;
- malignant tumors;
- excessively dense hymen or incomplete defloration;
- traumatic injury to the vaginal mucosa, candidiasis, or excessive dryness;
- abnormalities of the vagina.
Some of the fair sex talk about the phenomenon of bacterial vaginismus. In fact, this pathology does not exist, most likely, in this case we are talking about another disease similar in name - bacterial vaginosis , caused by the predominance of pathogenic flora in the body. However, if we consider that inflammatory processes in the vagina can provoke the development of vaginism, it is quite likely that some women call this form bacterial vaginism. In fact, bacterial means, caused by certain bacteria, and vaginismus itself, as a pathology, is not caused by bacteria. However, bacterial vaginosis can trigger the development of vaginitis due to the painful intercourse in this pathology, and the development of psychosexual disorders.
At the same time, bacterial vaginismus will be an incorrect name, therefore in medical practice this name does not apply to this pathology.
Note that being a consequence of psychogenic disorders, vaginismus itself leads to the development of various mental disorders of a woman. Without getting sexual satisfaction, neurosis and other pathological mental reactions can develop . In addition, psychosexual disorders can occur in her sexual partner, especially in cases where before the occurrence of this violation a woman lived with him a full sexual life.
CLINICAL PICTURE
This psychosexual violation is manifested by spasm of the muscles of the vagina, perineum, and the inner surface of the thighs, which makes it impossible to insert a gynecological mirror or penis inside. Symptoms of the disorder are complemented by severe pain.
There are three degrees of this disorder. The first degree is when the symptoms are little pronounced and the spasm occurs only in the case of the initial stage of the penis insertion. When the second degree - spasm develops not only with the introduction of the penis or gynecological speculum into the vagina, but even with simple touches of the genitals. And the most severe form, the third, the symptoms of which are similar, but develop even at the thought of a woman about sexual contact.
As mentioned above, due to the inability of a woman to get a sexual discharge, her symptoms of psychosomatic disorders are increasing. In particular, she becomes irritable, gets tired quickly, she has mood swings and depression .
DIAGNOSIS AND FEATURES OF TREATMENT
One of the main questions with which women suffering from this pathology come to doctors is how to cure vaginismus? Treatment should be prescribed after a thorough diagnosis and examination of the patient to identify the causes of the disease. Diagnosis of the disease is carried out during a gynecological examination - when you try to enter the mirror, a spasm of the vaginal muscles develops, which makes examination in principle impossible. Anamnesis is also collected to determine the provocative factor and the cause of the development of the disorder. Symptoms and their severity gives the doctor an idea of the stage of the disease. At the same time, in order to clarify the symptoms, it is sometimes necessary to have a conversation with a woman’s sexual partner, since some factors may not be recognized by her.
To determine the cause of the secondary form of pathology, it is necessary to take a bakposev and a smear on cytology, to conduct an ultrasound examination of the pelvic organs of women with this disorder. In some cases, additional instrumental research methods may be required to confirm or exclude certain diseases that can cause vaginismus.
Gynecologists are well aware of how to get rid of vaginismus, so the treatment of this pathology does not present serious difficulties, but it must be comprehensive, and include not only special exercises and medical manipulations, but also psychotherapy. Sometimes psychotherapy alone is not enough, so the treatment of vaginismus is supplemented with the intake of certain psychotropic drugs that allow a woman to get rid of a phobia or a certain fear.
First of all, the cause of the pathology should be eliminated. In particular, if these are physiological factors, the necessary therapy should be carried out (antibiotics, anti-inflammatory, etc.). In particular, if vaginismus is bacterial, that is, it is provoked by infection of the genital organs, the disease must be treated with antibiotics, to which the microorganism that caused it is sensitive. And if the patient has a malignant tumor - resection and chemotherapy, etc. are shown. Healthy food Healthy food - recipes for every day! If we talk about medical manipulations used in the treatment of this problem, they consist in the gradual introduction into the vagina of dilators of different diameters, starting with the smallest. Extenders are left for 10 minutes so that a woman can get used to the sensations. If the doctor fails to perform the procedure, it is suggested that the woman herself inserts dilators into the vagina, which minimizes discomfort.
Women who have vaginismus can supplement the treatment with special gymnastics. These are the well-known Kegel exercises that train the muscles of the pelvic floor and allow it to master the skill of self-tension and relaxation of certain muscle groups.
